Creating plain 'comments' shouldn't be possible

A couple people have managed to create plain comments that aren't part of a wall or blog post. We should eliminate this possibility.

Is it a good question to ask about the context of the 'plain comments'? A link to an example, or screen capture? This seems like a good place to get started. thanks.
